TRANSCRIPT REQUEST FORM
Students and alumni can request official and/or unofficial transcripts using the request form below. Please allow 3-5 days for processing; 7-10 days for processing during registration periods or at the end of a semester. The transcripts are processed in the order in which they are received. Incomplete forms will delay your order.
Due to University Policy, we are unable to release transcripts (both official and unofficial) if a student’s record indicates financial, police/safety, and/or library holds.
If you'd prefer to print and mail a transcript request form you can download it here. All transcripts are mailed by United States Postal Service, first class. Special overnight/express mailing labels must be ordered and pre-paid directly by the student and can be ordered at https://www.ups.com. If this is a special mailing request, please indicate under “Special Instructions” on the transcript request form below.
Additional forms that need to be included with the transcript (i.e. LSAC, PharmCAS, PTCAS etc…) can be attached to the form below, emailed to a Registrar Office representative or faxed to the Registrar’s Office at 814-824-2172. If you will be providing a form that will need to be attached to the transcript please indicate under “Special Instructions”.
Unofficial (Student Copy) transcripts do not carry the seal of the university and are stamped “unofficial”. A student can order one complimentary copy. Unofficial copies can be mailed, faxed or emailed. Please indicate additional information under “Special Instructions”.
Official transcripts bear the Mercyhurst University seal and the signature of the Registrar. A fee of $10 is charged for the issuance of each official transcript or an undergraduate/graduate set. Official transcripts ordinarily are mailed directly to other institutions and agencies. Students can receive official transcripts directly, however, it will indicate on the front of the envelope that the transcript was “issued directly to the student and remains official if delivered in a sealed envelope”. Once the envelope seal is opened the transcript is no longer considered official. Due to the embossed seal, official transcripts cannot be faxed or emailed. Unpaid/Declined credit card transactions will be processed as a student copy.
