If you need to update your contact information, please complete “Name Change Request Form” and/or “Address/Telephone Update Form”.

Request Processing Instructions

  1. Download the Name Change Request Form above for Name change and/or the Address/Telephone Update Form above for Address/Telephone change.
  2. Print out the form.
  3. Complete the form (please print).
  4. Include your signature and date.
  5. Deliver, mail, or fax it to the Registrar's Office.
  6. For Name Change, MUST submit a copy of the official supporting document along with the form to the Registrar's Office.
