Name Change Request Form
- Download the Name Change Request Form above for Name change and/or the Address/Telephone Update Form above for Address/Telephone change.
- Print out the form.
- Complete the form (please print).
- Include your signature and date.
- Deliver, mail, or fax it to the Registrar's Office.
- For Name Change, MUST submit a copy of the official supporting document along with the form to the Registrar's Office.
